The Promotion and Disease Prevention (HPDP) Program Manager (PM) Registered Nurse delivers fundamental knowledge-based care to assigned clients while developing technical skills.
Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments as prescribed.
Assesses, plans, implements, and evaluates care based on age-specific components.
Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
The HPDP PM advocates, leads, or participates in policy, program development, and resource allocation to support HPDP programs and services for Veterans.
The HPDP PM is the primary point of contact for national health promotion campaigns and events, ensuring local implementation and participation.
Coordinates the development of user-friendly tools to assist in successful implementation of HPDP programs and services.
Collaborates with community partnerships and resources, and Whole Health. Provides leadership in the development or modification of service delivery processes to support the successful implementation of HPDP services.
Oversees the provision of staff education, orientation, competencies, and providing quality improvement consultation related to the program specifically.
