Afterschool pick up and care for 8 year old and 10 year old

We are a family of 4 plus a sweet black lab mix. Our 8 year old daughter and 10 year old son attend school in Cobble Hill and Park Slope respectively. Our home is in Prospect Lefferts Gardens.We need a caregiver who can pick our kids up from school, take them to their after school activities and then home, and oversee homework, chores, and piano practice...
