**Conviva Senior Primary Care** in **Palm Beach, Florida** is looking to add a new Primary Care Nurse Practitioner to our team. _We have openings at our offices in West Palm Beach, Jupiter, and Greenacres._

_**About the role:**_

- **Monday-Friday, 8-5 Hours** in an Outpatient Setting
- See **12-15 patients per day** under a value-based care model
- Team-based model with daily huddles
- Full support staff including care coaches, MAs
- eCW EMR system

_**Benefits:**_

- Base salary plus 20% quality incentive pay, for total earnings up to $150,000
- Sign-on bonus
- Occurrence-based malpractice
- Full benefits including 401k matching, CME time + funds, health and life insurance, etc.
- PTO + Vacation + CME + Volunteer time off

_**Requirements:**_

- Florida Licensure and Certification
- 2 years experience with geriatrics as a Nurse Practitioner
