Job Summary and Qualifications

  •  The Nurse Navigator's primary function is to support patients through complex healthcare environments for the optimization of care and outcomes. 
  • The Nurse Navigator facilitates patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex diseases, and streamline the intervention process. 
  • The Nurse Navigator guides patients and families through the healthcare system by assisting with access issues in an effort to expediate timeliness of care and treatment. Navigators achieve this goal by building relationships with patients and physicians, coordinating the defined plan of care, assisting with appointments, assisting to resolve barriers, providing education, and connecting with community resources.
  • The navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for patients, including support with interdisciplinary and cross-facility coverage and collaboration. 
  • The navigator is responsible for timely data collection that promotes tracking of the navigation program performance in line with HCA Healthcare, division and facility goals (growth, quality, practice guidelines, etc.).
  •  The navigator identifies opportunities to streamline care practice through evidence-based guidelines and ensures quality, patient-focused care.

What qualifications you will need:

  • Basic Cardiac Life Support (BLS)
  • Service Line Specialty Certification - Preferred  
  • One to three years of clinical experience required
  • Three plus years of experience as a nurse navigator and/or patient educator in service line-Preferred
  • Required Education: Bachelor of Science in Nursing or enroll in a BSN or advanced degree program and obtain a BSN within 5 years of hire date into RN position. Grandfathering will be accepted between departments and Mission Health entities. Exemptions, or extensions, may be approved by the facility CNO and reported out to the CNO Council members. Each facility will ensure tracking of exemptions should this information be needed. RNs with 10 years or more of experience are exempt. 
  • Required License/Certifications: Must have and maintain current licensure as a registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBON
