You will be providing care management and coordination in the ambulatory setting. As part of an interdisciplinary team, you will be central to working with Medicaid, Medicare and Commercially insured patients to support them in their care journey
Works collaboratively with the multidisciplinary healthcare team to include the primary care physician, specialists, nurse case manager, community health worker, pharmacist, to coordinate care for patients/families presenting with social, financial or biopsychosocial needs. This position is to support the referrals from care coordinators and providers in the ambulatory setting seeking biopsychosocial and behavioral health services for their patients. Identification, assessment and treatment of mental health conditions, such
as depression and anxiety;
Case management/care coordination, particularly for individuals with chronic and/or complex medical conditions; Patient navigation, especially for patients moving among different health care levels (e.g., inpatient, outpatient, home health, or long-term care); Counseling on adjustment to chronic disease, life planning and end-of-life issues; Identification and referral for specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support; Education and support programming (e.g., diabetes education, parenting classes, domestic violence support programs) for individual and groups; Assistance with entitlements, medications, transportation, and advance directives; Assessment and intervention in domestic violence and child abuse situations; Outreach and coordination with other community
resources and agencies; and Community-level advocacy on behalf of patients and families.
ASSESSMENT: Assesses patient's mental and social functioning by conducting biopsychosocial assessments and charting accordingly. Engages in assisting patients and families with life planning including clinical, social and financial management of resources through the end of life. Assists with advanced directives, enrollment in Palliative Care
Programming and end of life care. Provides appropriate referrals for patients as required and/or appropriate to community resources.
COUNSELING: this generalist position works as part of a
multi-disciplinary team, to provide Individuals/family supportas indicated by patient's needs to full age spectrum on a wide variety of patient issues.
PROGRAM DEVELOPMENT: May work in concert with the ambulatory
behavioral health team and other ambulatory providers to identify opportunities for outreach and support groups.
QUALITY: Participates in departmental and system wide performance improvement and quality initiatives. Maintain appropriate charting documentation per Loyola and Department Policy. Documents in EPIC all pertinent patient information by the close of each business day.
