Birchwood Health Care Center, a Lifespark Skilled Nursing Community, is looking for a dedicated and passionate Social Services Designee to join our growing team.

Birchwood Health Care Center is home to seniors requiring TCU (Transitional Care Unit) Care and Long-Term Care. We are in Forest Lake, near North Shore Dr and Forest Lake. We have been helping seniors Age Magnificently since 1979 and are part of Senior Care Communities, a local non-profit supporting seniors.

Schedule: Part-time, 20 Hours per week.

Pay Range: $22-24.72/hour

In this position you will be responsible for identifying and providing for each resident's social, emotional, and psychological needs, and the continuing development of the resident's full potential from admission to discharge. Duties include:

  • Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident.
  • Responsible for completing the MDS and care plans for new admits.
  • Orient residents to the long-term care environment and facilitate adjustment upon placement.
  • Assist residents and families with adjustment to changes, including room changes.
  • Assist families with the engraving and/or marking of resident's personal items.
  • Review residents' rights with residents and staff and attend to various personal needs of the residents.
  • Assist in the development of a volunteer program and assist in facilitating family group meetings.
  • Document progress notes that relate to each resident's care plan when necessary and within policy timeframe.

Preferred Qualifications:

  • Associate degree in social work or related field or two years equivalent experience in social services position in lieu of degree is preferred.
  • Must be able to relate professionally, positively, and cooperatively with residents, resident's families, and employees.
  • Must possess advanced computer skills.
