Summary

The primary purpose of this position is to provide case management. Case Management is a process of patient assessment, coordination of care, integration of services, and follow-up and evaluations to meet and assist with the mission of improving continuity of care and patient flow. This position is located in the Division of Nursing Services, Outpatient Nursing Department, Tsaile Health Center, Tsaile, Arizona.

Duties

Provides comprehensive nursing care to patient of all ages and their families who present with an unlimited variety of injuries and illnesses by counseling and teaching on preventive, curative and rehabilitative measures; documenting education activities and response of patient/families into health records; preparing and reviewing documentation of patient care; ensuring all care provided is documented in patient's chart; triaging for emergency treatment per status of patient and level of care needed; observing and initiating nursing interventions and therapeutic measures administered and escorting patient via ground to referral facilities as needed. Provides case management services and serves as a resource person to Patient Referral Services to multidisciplinary team, long term care, AHCCCS and other insurance or government services to coordinate health care needs. Educates, counsels, and supports and provides appropriate guidance in a crisis and intervenes or facilitates referrals appropriately. Conducts periodic assessments for care managed population. Evaluates desired and actual outcomes of action and care plans. Provides highly specialized care coordination services to patients with High Risk and At Risk, chronic conditions and other patients/family teams of all personality types and medical conditions.
