Vacancies exist for RNs interested in providing Primary Care in a busy outpatient setting. This direct patient care includes, but not limited to, Veterans requiring medical attention either through scheduled appointments and/or Veterans seeking walk-inc are. This unique department is patient centered and team driven. Our primary care strives to develop and provide a comprehensive plan of care for each individual patient and their family with a focus on preventative health and education.

Duties include performing the following;

Video appointments
Conduct Patient and Family education including management of chronic diseases
Participate in coordination and collaboration of patient care management with PACT team
Conduct healthy lifestyle coaching and education
Patient assessments and critical thinking
Procedural skills
Computerized records management
Customer service.
