The Central Iowa VA is hiring for a RN assigned to Rehabilitation and Extended Care.



Duties

The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.

The Contract Nursing Home (CNH) Outpatient Staff RN is a professional nurse with board-based clinical knowledge enabling them to effectively implement health promotion, prevention, and management for adult or older adult Veterans residing in the VA contracted nursing facility.

Major duties include, but are not limited to:

  • Conduct site visits to the contract nursing homes; audit charting, interview Veterans regarding their care, ensure quality care is being delivered.
  • Identify patient care needs in accordance with the established standards, policies, and procedures consistent with nursing practice.
  • Interacts with Veterans, family members, and an interdisciplinary team of professionals.
  • Coordinates nursing care for Veterans with specialized/complex needs in the nursing homes.
  • Be resource for questions and concerns related to the process for obtaining specialty care, therapy services, or equipment.
  • Assist with notification related to sentinel and adverse events.

Work Schedule: 7:30am - 4:00pm or 8:00am - 4:30pm, Monday - Friday
Telework: Ad-hoc, in accordance to agency policy.
Virtual: This is not a virtual position.
Permanent Change of Station (PCS): Not Authorized.
