Works in partnership with physicians to consider alternate levels of care if patient is not meeting clinical stability. Collaborates with Social Work to manage the needs of patients who have high acuity psychosocial needs coupled with need for medical care to mitigate risk for readmission. Responsible for performing job duties in accordance with the mission, vision and values of Tampa General Hospital. In addition to the above duties, the Heart Failure Care Coordinator provides effective clinical health coaching to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk.
* Graduate of accredited School of Nursing; Three years nursing experience in acute care or ambulatory setting.
