As our Joint & Spine Surgical Coordinator, you will a Case Manager Registered Nurse (RN), you will establish effective communication with physician offices, hospital departments, staff, and rehabilitation services to facilitate a smooth continuum of care for all joint replacement/spinal fusion patients and families. In addition to coordinating care with case management to anticipate high-risk patient issues, you will also assist in implementing an efficient discharge/rehab plan for joint replacement/spine patients. Developing education for orthopedic staff including treatment protocols and orthopedic inservices
Working with multidisciplinary team members on performance improvement initiatives.
Participating and coordinating (with the marketing department) community orthopedic educational programs presented by physicians.
Working cooperatively and communicating effectively with all team members to ensure coordination of care and documentation of services provided.

Graduate of an accredited school of nursing
Bachelor’s Degree preferred
