As the RN Case Manager for Maternity and/or NICU, you will have a panel of members with high-risk pregnancies and/or NICU babies with complex medical/behavioral needs. You will provide support and address barriers to care and SDOH with the goal to improve health outcomes.
To promote healthy pregnancy, delivery, and postpartum outcomes for both mother and baby, it is essential to provide evidence-based, trimester-specific, and postpartum education. This includes assistance in developing a birth plan, accessing value-added benefits, and understanding covered services, including Medicaid postpartum coverage. Additionally, support should be given for accessing contraceptive care based on family planning needs, conducting mental health and substance use disorder (SUD) screenings with referrals to treatment when necessary, and coordinating with obstetric and other providers for appointment assistance, communication, and advocacy.
This is a Field-Based Role with a Home-Based Office
If you reside within the Ellis, Rush or Trego Counties, KS, you will have the flexibility to work in both the field as well as telecommute* as you take on some tough challenges.
  Provide evidenced based trimester specific and postpartum education to promote a healthy pregnancy, delivery, and postpartum outcomes for mother and baby
Help member set goals and develop a care plan to achieve those goals
Proactivity engages the member to manage their own health and healthcare
Perform outreach to providers, hospitals, Members, and clinics
Attending community events 
Able and willing to travel within the assigned region per the business needs

2+ years of experience working within the maternal and child health population
~1+ years of experience with MS Office, including Word, Excel, and Outlook
~ Ability to travel up to 75% of time in assigned region to visit Medicaid members in their homes and / or other settings, including community centers, hospitals, or providers' offices
~ Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
~ Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
