Conduct outreach to patients from diverse populations to engage them in population health initiatives
Build rapport and establish trusting relationships with patients to promote their active participation in care
Identify barriers to healthcare access and address social determinants of health that impact patient well-being
Educate patients on available resources, preventive measures, and self-care strategies
Perform comprehensive medical and behavioral assessments to evaluate patients' physical, psychological, and social needs
Identify and address mental health issues, substance abuse, domestic violence, and other psychosocial factors
Collaborate with healthcare providers to develop individualized care plans based on assessment findings
Advocate for patients' needs and preferences in care planning and decision-making processes
Collaborate closely with a multi-disciplinary team, including physicians, nurses, care coordinators, and community partners
Participate in care conferences and team meetings to discuss patient cases and develop integrated care plans
Coordinate referrals to community resources, mental health services, and support groups
Maintain accurate and up-to-date documentation of patient interactions, assessments, and care plans
Collaborate with healthcare providers to ensure continuity of care and coordination of service

Master's degree in Social Work (MSW) from an accredited institution
Knowledge of medical and behavioral assessment techniques, mental health interventions, and crisis management
Strong understanding of social determinants of health and their impact on patient well-being
Strong interpersonal skills to build effective relationships with colleagues, patients, and community partners
