Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Full-Time Speech Language Pathologist at The Lakes of Sylvania , a dynamic and innovative Skilled Nursing and Assisted Living health campus located in Sylvania, OH . You must hold an active or pending Speech Language Pathology (SLPCCC) state license to apply for this position.
Responsible for providing a full range of speech-language pathology services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. Demonstrates accountability for and contributes to program development, quality improvement, problem-solving and productivity enhancement in a flexible interdisciplinary environment.
Provides therapy services including initial assessment, treatment planning and therapeutic intervention in campus locations, home health and/or outpatient care.
• Documents the course of patient care including progress made and continuing need for treatment.
• Functions as a resource for and provides employees with in-service/cross-training on speech therapy knowledge and skill areas.
• Reports any complaints and/or grievances made by the residents to the Rehab Program Director or Director of Health Services. Reports incidents or suspected incidents of resident abuse to the Rehab Program Director/Director of Health Services and/or the Executive Director immediately.
• Travel, as needed, to other campus locations and/or to home health assignments in the designated territory.
• Complies with all resident rights per the company's Resident Rights Policy.
• Participates in survey preparation activities.
• Must hold an applicable, unencumbered Certificate of Clinical Competence from the American Speech-Language-Hearing Association or be CCC eligible and hold current, unencumbered state license.
Graduate of a master's level program in speech-language pathology which is either accredited by the Educational Standards Board -or- whose course work and practice meet ASLHA certification requirements.
Current CPR Certification for healthcare providers through CPR training that includes hands on practice and in-person skills assessment preferred.
Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
