The Social worker is responsible for superior practice and/or the assumption of additional or special responsibilities. Evaluate case data. Perform treatment processes. Provide social work interventions based on assessment of resident’s need and case service plan. Conduct assessments, unit inspections, and complete clinical documentation as required in a timely manner. Document all resident contact in progress notes. Conduct services with residents using evidence base practices specifically Critical Time Intervention. Complete all intake assessment and coordinate intake process for new residents. Prepare integrative Person Centered Service plan, based on assessments from referral source and in collaboration with resident. Utilize Agency resources as well as community resources to perform related tasks. Maintain an up to date clinical charts at all times. Other duties as assigned. Qualifications: The Jewish Board is currently looking for talented professionals of all cultures, religions, races, and gender expressions with the following qualifications: Master’s Degree in Social Work (MSW) and LMSW obtained within 6 months of hire. Previous work with individuals with severe and persistent mental illness and substance abuse a plus.
