Job Description: RN Hospice Home Care Baylor for: either Friday, Saturday, Sunday or Saturday, Sunday, Monday 24 hour position/paid for 40 hours to provide hospice and palliative care at home.
· Support our patients, families, and loved ones, as they go through their end of life journey 
· Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case managementstrategies
· Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families. 
· Utilize state-of-the-art technology to document skilled services provided.
Homecare or Hospice experience preferred.
· Computer proficiency required.
