Registered Nurse-Shift Coordinator- Weekends

Responsible for providing quality education, follow-up and emotional support to the population and their families. Case manages the population throughout the inpatient and outpatient settings and measures outcomes to maximize continuity of care between the settings. Manages projects and programs related to clinical management, performing quality improvement activities to measure program processes and outcomes.
Provides ongoing review and development of educational materials related to clinical management.
Develops and manages clinical paths as appropriate, and measures outcomes of care to maximize continuity of care and appropriate resource utilization and length of stay. Coordinates multispecialty team clinics.
Provides direction to specified projects/programs pertaining to the population, assessing, planning, developing/maintaining, implementing and evaluating outcomes of same.
Documents educational process/patient information according to program/MDPH guidelines.
Communicates, collaborates, networks with and acts as a consultant to clients/significant others, other members of the health care team, and the community in order to ensure continuity of care and coordination of services, and to provide needed resources to the community.
