The Transplant NP practices as an independent care provider in collaboration with the transplant physicians, including the Chief of Transplant, primarily on the inpatient Renal & Transplant Unit and the Transplant Short Stay Unit and out-patient settings. Her/his practice is characterized by comprehensive health assessments and management of patients with acute and/or chronic renal disease. Acts as liaison between Saint Barnabas and all referring dialysis centers, community agencies and referring physicians. Ensures continuity of care through effective communication and collaboration with the multi-disciplinary team in all areas of the transplant department. 4*10hr shifts weekly with-rotation to weekends.

* Obtains health histories, examines and treats potential transplant candidates, transplant recipients and living donors primarily on the in-patient Renal & Transplant Unit and as requested in the pre and post transplant outpatient settings under the direction of the collaborating transplant physician. This may include appropriate referrals to endocrinology (i.e. steroid induced diabetes); surgery (i.e. lymphocele drainage); cardiology (i.e. clearance for transplant candidate); urology (i.e. stent insertion/removal) etc.
* wound care; monitoring weight; adjusting diuretics/monitoring electrolytes etc.
* Acts as a liason between transplant coordinators, transplant physicians, transplant consultants and ancillary staff in providing continuity of care to patients and families.
* Participates in the development of appropriate clinical protocols and related policies and procedures.
