​The Oncology Service Department at the Elliot Hospital takes great pride in serving their community, patients, and families. If you are a Social Worker with strong clinical experience/skills and you love to care for patients, come and join our incredible team

The Elliot position supports the Solinsky Center for Cancer Care in Manchester and Londonderry, including Medical Oncology, Radiation Oncology and Breast Health Services, providing ad-hoc support services and support groups for patients.

Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of each patient
Identifies relevant community resources and links them to patients as appropriate
Coordinates with key personnel at all community health facilities, which treat oncology patients, to ensure patient needs are met
Coordinate’s homecare referrals, acute rehab, and nursing home referrals as defined
Serves, as a resource/liaison to community social service agencies relative to identified patient/family needs
Master of Science in Social Work, Required
One (1) to three (3) years of experience in working with Oncology patients and cancer survivors, Preferred
