The Cardiovascular Case Manager is responsible for planning, coordinating, negotiating and initiating the efficient movement of the patient through the continuum of care, based on medical needs, functional ability and available resources. Assesses the patient continuously for medical necessity, appropriate level of care, financial situation and discharge barriers.
Initiates discharge planning and will collaboratively work with the interdisciplinary team to identify and coordinate needed services and resources.
Provides education to patient's family and staff as required.
Audits data abstraction for accuracy and provides concurrent feedback to physicians regarding quality measures The CV Case Manager reports to the Lead CV Case Manager. This position may have additional or varied physical demand and/or respiratory fit test requirements. Potential for exposure to infectious diseases, bloodborne pathogens and other infectious materials as defined by OSHA. RN experience should include 5 years clinical experience in an acute care environment OR 3 years' experience with a bachelor's degree in Nursing.
Cardiology experience strongly preferred.
Must be able to communicate effectively in English.
