Grounded by our belief that outstanding care is best delivered in a team-based environment, our home health physical therapists collaborate to provide management and delivery of patient care plans.

This position supports patients in Toledo, OH and surrounding areas and is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year
Up to 22 paid holiday and personal days off in year one

Bachelor's degree or Master's degree in Physical Therapy from an accredited college required
* Minimum of one year healthcare experience
* Demonstrated experience with tablets, mobile phones and EMR software

Includes compensation related to patient care visits, non-visit activities, paid days off, shift premium pay, and first year success bonus.
