Description The RN Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. Practice is within the scope defined by the Washington State Nurse Discipline Practice Act, based on the nursing process model and concept of community health nursing therapy.

~2 years full time experience practicing as a registered nurse in a Long Term Care, Medical/Surgical Unit, and/or Skilled Nursing Facility.
~1 year recent, hands-on technical clinical experience (occurring within the last three years) in a Medicare-certified Homecare or Hospice agency/facility doing intermittent visits and case management.
~ Recent hands-on experience in wound care, venipuncture, ostomy, catheters, and feeding tubes.
~ Rehab nursing experience.
~ Experience with office software programs including Outlook, Word, and/or Excel.
