Aviator Home Health is looking to add highly skilled, dynamic therapy staff to our proven, evidence-based methodology and trusted philosophy with special emphasis on delivering the highest level of skilled home health care.
Aviator is changing the culture and experience of home health for the better With Aviator, you're not just a clinician out in the field, you'll be part of a CLINICAL TEAM that works together for the betterment of our patients with unparalleled support from our office and management staff.
Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning, including OASIS assessments at appropriate time points and appropriateness of patient for home health.
Provide therapeutic treatments and evaluation of equipment needs as applicable.
Evaluate outcomes of care with appropriate discharge planning.
Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
Promote Agency philosophy and administrative policies.
Bachelor of Science in Physical Therapy or higher from an American Physical Therapy Association approved program.
Skills: Therapy skills as defined as generally accepted standards of practice. Proof of current BLS for healthcare workers CPR. Knowledge of durable medical equipment.
Experience: Home Health and manual therapy experience preferred, HomeCare HomeBase knowledge a plus.
