The Nurse Practitioner or Physician Assistant applies advanced education and clinical competencies to achieve optimal patient outcomes. The Nurse Practitioner or Physician Assistant provides health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions. Advises executives to develop functional strategies (often segment specific) on matters of significance. Exercises independent judgment and decision making on complex issues regarding job duties and related tasks, and works under minimal supervision, Uses independent judgment requiring analysis of variable factors and determining the best course of action.
a Bachelor's degree
* 1 or more years of adult medicine experience as an NP or PA
* Fully engaged in the concept of "Integrated team-based care" model
* Willingness and ability to learn/adapt to practice in a value-based care setting
* Bilingual (English and Spanish) is a plus
* Knowledge of HEDIS measures and quality indicators
* Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment
