Note: The Centers for Medicare & Medicaid Services (CMS), in collaboration with the Centers for Disease Control and Prevention (CDC), require COVID-19 vaccinations for all Medicare and Medicaid certified providers. Schedule- Monday through Friday 1pm to 9pm with occasional on call requirements.
This position works out of our office in Fairfax VA, can be remote after training which would take place Monday through Friday 8am to 5pm.

As a RN Clinical Field Staff Liaison (CFSL), the work you do every day makes a difference in the lives of our patients by ensuring quality and safe delivery of home health services.

You’ll provide support and act as a resource to clinicians in the field. You will assist in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management.
Operations: Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; Monitor Home Health Aide supervisory visits.
Patient Care: Work in the field with staff and patients as needed.
Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
Minimize Patient Risk: Ensure Start of Care compliance and quality and safe delivery of home health services. Implement quality improvement programs in the field.
Policies: Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
Teamwork: Assist with monthly Case Manager, Nurse and Therapy meetings. Minimum one-year home health experience is preferred
Minimum of Three (3) years of experience as an RN in an acute care setting
Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
