Halcyon Home is a fast-growing, central Texas-owned home health, hospice, and home care company. We emphasize hospitality and compassion to encourage positive patient outcomes for our refined, aging clients. We were voted Best in Home Health/Hospice four years running and Best in Home Care three years running by the Senior Resource Guide. A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus
Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.
oversees other team members, when applicable, in the performance of patient care that is consistent with patients' needs.
Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
Instructs on use of equipment such as walkers, wheelchair, canes, etc., Utilizes appropriate therapy equipment specific to patients' diagnoses and needs.
