Responsible for helping to build and support a heart failure guideline directed medical therapy titration clinic providing both face-to-face and virtual services to heart failure patients with the goal of rapid medication titration and evaluation for device or advanced heart failure therapies if indicated.
Cover in our longitudinal heart failure and heart success clinics as needed providing care to patients who have advanced systolic heart failure and/or diastolic heart failure from ischemic, nonischemic, infiltrative, structural, congenital or various other cardiomyopathies..
Responsible for helping to build and support a heart failure guideline directed medical therapy titration clinic.
Willingness to work five 8-hour shifts; No weekends, call or night shifts required.
Multidisciplinary team comprised of physicians, APPs, nurse navigators, clinic nurses, clinical pharmacists, social workers, and various other support staff across 3 facilities.
Mechanical circulatory support, including LVADs, BiVADs, Impella, IABPs, ECMO
Board certified nurse practitioners and/or physician assistants specifically trained and proficient in providing cardiac services
~ Cardiology experience required; Masters Degree or Doctor of Nursing Practice Required.
