We are looking for a MDS Coordinator RN for this skilled nursing home that serves both active sub-acute and LTC population. This position would be the department manager and would supervise and oversee management of the department and other MDS staff members.

* Responsible for developing, implementing, and coordinating the Resident Assessment Instrument (RAI) process in a long-term care and Short-Term Rehabilitation / Skilled Nursing Center
* Ensuring the center stays in compliance with current professional practice standards, physician’s orders, company policies and procedures, and local, state, and federal regulations, as well as providing oversight of the center’s MDS Coordinator.
* The coordinator will also be responsible for the overall PDPM process and meetings as well as completing assessments, CAA’s, care plans and assisting to ensure correct coding for the QMs as well as ensuring timeliness, accuracy, and transmission of the department MDS’s.
*Associate's Degree in Nursing (ASN) or above required.
* Requires experience in clinical reimbursement as it relates to MDS completion, accuracy and transmission.
* Knowledge of the MDS, RAI process and an understanding of Medicare regulations with regards to PDPM and OBRA
* Responsible for the overall PDPM process and meetings as well as completing assessments, CAA’s, care plans and assisting to ensure correct coding for the QMs as well as ensuring timeliness, accuracy and transmission of the department MDS’s.
*Jacksonville Nursing & Rehab, located at 4134 Dunn Ave., Jacksonville, FL 32218, is a 163-bed, joint commission accredited nursing center that offers in-patient residential care, short-term rehabilitation, and specialty services. Our property features private and semi-private rooms, landscaped grounds, 7-day-per-week admissions, residential activities, and a rehabilitation gym with inpatient and outpatient therapy, and accept a wide variety of insurance plans.
