We are seeking a compassionate and skilled RN Case Manager to collaborate closely with clinical teams and provide comprehensive case management services. In this remote role, with occasional travel to clinical sites in the Philadelphia and Harrisburg, PA areas, you will play a pivotal role in optimizing patient wellness and satisfaction.
What You'll Be Doing
Conducting thorough assessments of patients' healthcare and psychosocial needs.
Collaborating with patient navigators to refine patient interviews and develop individualized care plans.
Implementing and maintaining personalized care plans in alignment with internal protocols and accreditation standards.
Delivering evidence-based clinical interventions to support patients' care plan goals.
Partnering with patient navigators to address non-clinical aspects of care plans, such as scheduling appointments or arranging transportation.
Providing ongoing support and outreach to patients, continuously monitoring their progress and adjusting care plans as needed.
Developing escalation plans with primary care physicians for acute, non-emergent exacerbations of chronic conditions.
Educating patients on their conditions and treatment options.
Advocating for patients and facilitating access to necessary services and resources.
Following up on referrals and maintaining accurate documentation in electronic medical health records.
Utilizing evidence-based clinical protocols and quality metrics to ensure optimal patient outcomes.
What You'll Need To Have
Active Registered Nurse license in good standing.
3+ years of experience in RN Case Management in an outpatient setting.
Preferred certification in Case Management (CCM), or willingness to obtain within one year of employment.
Supervisory experience is desirable.
Strong written and oral communication skills for effective interaction with patients, families, and interdisciplinary teams.
Proficiency in computer skills, including familiarity with multiple EMR and Case Management documentation systems.
Ability to meet accreditation and quality standards, including NCQA and HEDIS.

Bilingual proficiency in English and Spanish is preferred.
Compensation
We offer a competitive compensation package that goes beyond just base salary. Our benefits include comprehensive medical, dental, and vision plans, long and short-term disability, life insurance, and a 401k plan with a generous match. Additionally, our paid time off program includes paid holidays and flexible time to maintain a healthy work-life balance.
The base pay range for this role is between $80k to $85k per year, paid bi-weekly according to our standard payroll practices. Final compensation decisions are based on various factors, such as skill set, experience, education, location, and licensure/certifications.
