We are seeking a dedicated Licensed Practical Nurse to join our healthcare team. The ideal candidate will provide quality patient care in various settings, including inpatient facilities, urgent care centers, and home community care.

Responsibilities:

- Deliver exceptional patient care with a focus on compassion and professionalism
- Conduct discharge planning and coordinate post-hospitalization care
- Perform venipuncture and administer medications as prescribed
- Provide specialized care in areas such as ophthalmology and durable medical equipment (DME)
- Assist with urgent care situations and respond promptly to medical emergencies
- Collaborate with healthcare professionals to ensure comprehensive patient treatment
- Support patients of all ages, including experience in pediatric care

Qualifications:

- Valid LPN license in the state of practice
- Proven experience in patient care, preferably in diverse healthcare settings
- Proficiency in venipuncture techniques and medication administration
- Knowledge of discharge planning procedures and home community care protocols
- Ability to lift and move patients safely when necessary
- Familiarity with ophthalmology procedures is a plus
- Strong communication skills and the ability to work effectively in a team environment
