We are currently looking for a full time, Case Manager (RN) Registered Nurse in Wilmington, DE. 
Assesses the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the individual
Coordinates assessment, treatment planning, and return to work processes with the employee, providers, and employer
Coordinates and provides care that is safe, timely, effective, efficient, equitable, and patient-centered
Maintains collaborative working relationships with other community providers to develop specialized services as required
Promotes quality and cost-effective interventions and outcomes
Provides consultation to staff on specific cases
Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required
Certificate from an accredited Nursing School or Associate Degree (A.S.) Bachelor's Degree (BSN) from four-year college or university preferred.
Current hands on training in AHA or ARC Basic Life Support for health care providers is required. Advanced Cardiac Life Support may also be required based on contract scope of services.
Case Management (CCM) Certification, AANC – CM, American Academy of Case Management (AACM), COHN-S/CM, Certification in Occupational Health Nursing, Certification in Emergency Nursing, or Certificate in Case Management preferred.
At least three years’ experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic.
Occupational health experience required.
Knowledge of Internet software, Spreadsheet software and Word Processing software.
Knowledge and experience with Electronic Medical Records preferred.
Knowledge of workplace health and safety concepts and OSHA regulations preferred.
