You will be responsible for educating them and their families on prevention and healthy habits. As a Registered Nurse, you will: Report to the Clinical Manager on a daily basis
Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges
Develop the Physician Plan of Care in coordination with all health care professionals assigned to the case;
Provide documentation using a TABLET within the required 24-48 time frame
Be knowledgeable in the use of the current OASIS D1 assessment for all patient admissions, re-certification, transfers and discharges
Ensure that all home care services are provided in a high quality ROYAL manner and in accordance with Department of Health and Government regulations;

Westchester Qualifications: Proven experience as a registered nurse
Valid Nursing license
Pediatric experience
Must have a Vehicle and can drive
Must have experience completing comprehensive oasis assessments
Experience in acute care in a hospital setting or home care experience.
