Provide basic case management services to medically fragile children who are at risk of institutionalization and whose health, safety, and well-being can be maintained in his/her primary residence. Coordinate services with RN to ensure quality care.
Location: This position will service Cumberland County.
Pay: $30-$32 per hour
**Flexible Schedule / Mostly Remote Work / Community Based Services
Evaluate and document client programs
Conduct interviews and work closely with clients, families, other relatives, school, medical providers, and other community agencies contacts to obtain information to be incorporated into a social history which will aid in planning for required services as appropriate to the individual plan of care.
Graduation from an accredited School of Nursing. Ability to direct and carry out prescribed medical treatments and therapeutic programs.
Thorough knowledge of professional nursing theory and practice. RN certification with one year of nursing experience preferred.
Valid NC driver’s license and a clean driving record (this is a condition of employment) Personal auto liability insurance is mandatory if transporting clients in private/personal vehicle(s). OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities)
Drug testing
Education verification and other credentialing based on position requirements.
Positions that require driving Proof of driver's license, driver's insurance, and vehicle, IF required for providing transportation for individuals.
