requires travel throughout the Pioneer Valley. Some responsibilities can be completed remotely.
We're seeking a compassionate RN/BSN to join our Adult Foster Care (AFC) program. Step back from direct medical care to focus on personalized care plans and building genuine relationships with our members Enjoy flexibility of home and community visits to make a lasting impact and reignite your passion in AFC nursing. Provide comprehensive case management, assessments, and ongoing support for members’ health, medical, and safety needs
Develop, review, and maintain Plans of Care and Health Care Records
Offer training and technical assistance on health-related matters to caregivers and UCP employees
Consistent monitoring and documentation of caseload
Perform quality assurance reviews as required
ADN RN, BSN preferred from accredited program recognized by the Massachusetts Board of Registration in Nursing
RN License in good standing with the Massachusetts Board of Registration in Nursing
At least two years’ recent experience in direct care of the elderly or disabled adults.
Valid driver’s license, availability of private vehicle, up-to-date car insurance
Ability to work flexible hours to respond to the needs of members and families
Ability to travel regionally to conduct work in homes and community settings.
Comfortable with technology, remote applications (ex. and Microsoft Office
Satisfactory CORI, SORI, DPPC and National Fingerprint Background Check
