As a Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients. Requirements: * LPN licensed in the State of Delaware, or an appropriate State based upon work to be conducted within the State of Delaware. * 1 year experience in home health, hospice or care coordination preferred * Knowledge of ChristianaCare HomeHealth service lines.
