Palliative Care Nurse Practitioner - full-time
Trillium Palliative Services (a partner with Faith Hospice) has a unique opportunity for a Nurse Practitioner to join their amazing team on a full-time basis.

Not exclusive to hospice patients, our Trillium Palliative Services provide relief and comfort to anyone suffering with the symptoms of a serious or chronic illness – helping relieve pain, shortness of breath, nausea, loss of appetite, fatigue, depression, sleep problems, and treatment of side effects. Palliative care can be administered as needed by our team of certified and specialized palliative care physicians, nurses and nurse practitioners to help those affected cope physically and emotionally. We can also help guide caregivers and family members through the complex medical system to better understand the care options available.
We are adding this unique opportunity to the Trillium Palliative Services team as it continues to grow
Follow up care of patients followed by the Palliative Team
Participate in the weekend hospital rounding rotation one weekend every 6 weeks
Comfort with navigating various technologies and computer applications
Passion for high quality palliative care
Comprehensive knowledge and well-developed skills in medicine and pharmacology, pain and symptom control, psychology of loss, and understanding and acceptance of hospice principles of care
Hospice or palliative care experience preferred.
