Ascension Medical Group is looking for a Medical Director of Quality and Population Health to join our team. This position will be responsible for growing our primary care network across AMG Wisconsin. Department: Quality and Population Health

*Travel, PRN, Short-Term Option, etc.). Plan and direct medical policies and programs. Define and achieve financial targets in support of business goals; evaluate organizational functions and structure to best determine the allocation and utilization of services.
Ensure that clinical programs are in compliance with governmental and regulatory requirements.
Implement utilization review and quality improvement systems. May provide medical services to patients in an assigned area.
Consult with planning and operations leadership in new program development. Supervise any assigned medical/resident programs.
The Medical Director of Quality and Population Health will have the responsibility for planning, directing, and coordinating initiatives around:
Quality & Safety in alignment with Ascension WI CMO
Branding and Marketing
They will work collaboratively with physicians, system and regional leadership, hospital leadership teams, and staff to ensure our medical group meets its goals and objectives. The position reports directly to the CMO of AMG WI and will partner with their operations dyad partner.
5-10 years of progressive leadership experience in population health management as well as quality.
Excellent physician relations skills and proven experience growing service line volume in a multi-facility health system.
Skills in relationship development and collaboration across a matrix environment to establish a shared vision and strategy for quality and population health
The ability to develop short and long-term goals and plans; and budget preparation and ensures the services and programs are provided by the department to achieve quality standards.
Demonstrated ability to analyze market research data designed to provide relevant information for decision-making and strategic planning.
A leadership style which emphasizes personal accountability and visibility within the system and within the community.
A firm commitment to an understanding of delivering high quality healthcare services with an unparalleled commitment to customer service as a necessary component for success.
knowledge of population health operations in managed care, including quality management, outcomes, provider network, claims, financial management, care management policies, disease state and population management, members’ rights and responsibilities, and the ability to quickly understand and assess program compliance.
Licensure / Certification / Registration:
Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
Podiatrist credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) or Doctor of Dental Surgery (DDS) or Doctor of Dental Medicine (DMD) or Doctor of Optometry (OD) or Doctor of Podiatric Medicine (DPM) required.

The ideal candidate will be a clinical leader and content expert in quality measures, population health operations, case and population management, and experience working with a clinically integrated network and ACO. The candidate must exhibit strong team leadership, project management and organizational skills, as well as credibility with clinicians and medical group leadership. Is able to create and execute the implementation plan to deliver a strong care management model and engage network providers in positive trends and improving overall population quality and cost outcomes. Works with key members of the population health management team on the ongoing development and implementation of operational processes, strategic initiatives, policies and procedures used in delivery of care management services, as well as the ongoing integrated population analytics to manage and drive care management and other PHM workflows.
Focus on the ongoing management and outcomes driven performance improvement of the population health initiatives/programs in conjunction with the operations dyad director and other clinical teams. The dyad team is responsible for developing and implementing multi-disciplinary performance improvement teams that focus on identifying root causes to lagging performance metrics and then identifying, prioritizing, and implementing solutions aimed at improving program outcomes.
Focus on resolving staff issues, completes performance reviews, assists in the development, implementation and monitoring of quality and compliance activities, assists in the reviewing and revising department standards and policies and coordinates the orientation of new employees to the department. Will continuously work at identifying gaps in staff education and work with staff to close gaps.
