Territory : This position will work in the field helping patients in their home setting. Our office is in Roseville, MN and the service area of where our patients live is in the 13 County Twin Cities Metro Area. The general territory for this position would be the Southeast Metro (the cities that are south of highway 94 and east of 35W).

Able to travel within geographic area serviced by the office. Participate in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s).

Counsels and educates the patient and family in meeting healthcare related needs.
