~ Previous Experience in home care and home health. Knowledge of managed home healthcare is preferred
Solid computer proficiency in Microsoft Office including Excel and web based application
Involve other community agencies in the referral process as needed.
Compile patient information from referral sources including specific patient demographics, insurance and/or payor sources to ensure quality of communications, information and care for patients.
Provide skilled nursing care to patients in accordance with Physician’s orders in compliance with Federal, state and company regulations.
Request additional information for referrals that are incomplete and document requests in the clinical record.
Complete clinical record review/audits
Manage and document phone calls from physicians, clinicians, patients, referral sources and communicate patient updates/new orders to clinicians.
Follow up on labs and other clinical diagnostics test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow up on needed plan of care modifications; Review clinicians visit notes to ensure timely, complete, appropriate, and accurate submission of documentation by visiting staff.
Review, evaluate, and supervise service delivery to ensure appropriateness of care and utilization of services, equipment and supplies through activities such as medical records review and case conferences.
