POSITION SUMMARY
RN Case Manager

POSITION DUTIES
This position will coordinate care for patients in several remote locations for chronic care appointments, follow up for recent hospitalization, and an advocate to patient needs. Must be culturally sensitive, respectful and courteous.

MINIMUM REQUIRED QUALIFICATIONS
Experience in Care Coordination/Case Manager required.

LENGTH OF ASSIGNMENT

M-F 8:00a-5:00p
