Provides continuous comprehensive medical and nursing care, including preventive and supportive health
care services to patients and families.
* Manages health problems and coordinates health care for various patient populations. Consistent with the established nursing practice, differentiates between normal and abnormal findings in
the screening process to detect changes in health status that require physician's intervention.
* Develops and maintains partnerships with community-based organizations and makes referrals to
consulting physicians and/or other health care professionals and community agencies, as necessary.
* Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.
* Consults with physicians and other health care practitioners in the evaluation of health screening findings to
Formulates differential diagnosis and the plan of care based on medical history, physical exams, nursing
Identifies health and medical risks and needs.
* Develops, implements, and evaluates patient plan of care. clinical standards and practice protocols, where needed, to achieve medical and health goals.

A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
