Employee Assistance Program Seeking a Qualified Professional to work with adults as a part of a Community Support Team. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore an individual’s ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the individual’s community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal. Job Duties include: Assists with the development of relapse prevention and disease management strategies
Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations
Provides intensive case management
Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services
Assist with the individual’s housing search including engaging landlords to rent to individuals and writing reasonable accommodation letters
Assist with connecting individuals to financial and in-kind resources to set up and maintain their household Education and Experience Required: A bachelor’s degree in Human Services, Social Work, Special Education, or other closely related Human Services area
Two (2) years of professional experience with the adult mental health population Resume must include degree earned (major & minor), graduation month & year, relevant employment dates including month and year, and populations served. A Caring Alternative, LLC is a CARF accredited service provider with facilities in Asheville, Hickory, Marion, and Morganton, NC. A Caring Alternative, LLC is CABHA certified and a Trauma Informed Agency. We provide an array of services to Alexander, Buncombe, Burke, Catawba, Caldwell, Iredell, McDowell, and Mitchell counties which consist of Assertive Community Treatment Team, Community Support Team, Day Treatment, Integrated Care, Intensive In-Home, Medication Management, Outpatient Therapy, and Therapeutic Foster Care.
