ChristianaCare HomeHealth is seeking a Part Time 20 hours a week RN Liaison . As a Licensed Practical Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and/or alternative settings. 1 year experience in home health, hospice or care coordination preferred
~ Ability to work and travel to post acute care settings
