I. Nursing
Practice and Patient Care Activities A. Assesses physical, psychological and social
dimensions of the patient. B.
Assesses the patients’ and the
families’ need and readiness for
instructions in illness/disease control or management during hospitalization
and discharge planning. 1. Applies
knowledge of illnesses, injuries, diseases, and the psychology of human
behavior and uses available resources in the assessment process. 2. Provides
a written assessment of the patient on admission and subsequently in the
nursing process. 3. Summarizes
the assessment data and makes a nursing diagnosis, which
is charted and communicated to other patient care providers. C. Plans
a written plan of care for patients 1. Initiates
plan(s) based on nursing diagnosis, knowledge of specific illnesses, injuries,
diseases, human behavior and appropriate standards of care. 2. Documents
specific nursing orders, develops short- and long-term goals specifying
measurable objectives following intervention. 3. Utilizes
all available resources ( i.e ., clinical specialists, Head Nurses,
physicians, in-service coordinator, etc.) in developing the plan of care. 4. Updates
the plan of care as necessary based on continuing assessments of patient. 5. Develops
a discharge plan which uses all available resources. D. Implements
plan of care 1. Implements
clinical and technical aspects of nursing care in accordance with established
policies and procedures. Intervenes with
proper technique, procedure and safety precautions to meet individual needs of
patients. 2. Implements
the medical plan through administration of medications and intervention with
physician’s orders. 3. Documents
nursing intervention, patient response, treatment effectiveness, complications,
etc.; communicates information to peers, physician, and other members of the
health team. 4. Uses
all available in-house resource personnel and departments for treatment. 5. Treats
the patient and the family with dignity and respect, keeping information about
patient’s personal and social status confidential. 6. Functions
within the limits of State Nurse Practice Act and hospital and nursing
department policies and procedures.
Provides care and demonstrates skills comparable to a peer with similar
training and experience functioning under similar circumstances. E. Evaluates
nursing care provided 1. Reviews
and evaluates the effectiveness of intervention and implementation of plan. 2. Determines
the level of accomplishment of short- and long-term goals and objectives
following intervention. 3. Reviews
and evaluates personal nursing practice as compared to standard of care and
original plan of care.
