· Social workers provide psychosocial care in the hospital setting. Social workers link patients with community and agency resources. Assist providers in determining appropriate patient dispositions and facilitate referrals to treatment and community resources post-discharge 3. Provide assistance with assuring safety for behavioral health patients (i.e. managing development and delivery of policies, managing staffing needs in coordination with charge nurses and directors, etc.)Participate in hospital and community initiatives which address mental health and substance abuse issues 6. Fill in as a case manager/discharge planner, providing care coordination and discharge planning for primary medical patients on med/surg floors and in ICU (i.e. arranging aftercare services such as home health and SNF)
