This is some information about the opportunity we're offering, and you may be searching for:* Monday-Friday, day shift, 40 hrs. workweek, office environment and community-based work, with company vehicle provided for this, business casual to casual attire and more.
The *Licensed Practical Nurse (LPN*) as part of the ICM/ACT multidisciplinary team provides the coordination of both the behavioral and physical health care needs of the program participants, monitors participants medication and provides psychiatric nursing services consistent with physician’s orders. Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
* Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
* Provides ongoing assessment of participant’s mental health symptoms and responds to recovery plans.
*or participating in the Nursing Licensure Compact.*
* Knowledge of mental health systems and supports.
* Local travel required, including transporting participants.
