RN Case Managers are an essential part in quality care and coordination for our patients. Oversee the implementation and ongoing assessment of the patient’s plan of care
* Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.
* Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.
* Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
* Provide supervision of aides and nurses.
Company Fleet Vehicles - (complete with a gas card, maintenance repairs and inspections, and insurance coverage)
* Continued education programs to become certified in Palliative/Hospice care
* Flexible schedules
* A united team approach focusing on high quality care
willingness to travel.
* One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting.
*Advanced is willing to provide Employees with three (3) options in relation to a Sign-On Bonus. Advanced agrees to pay Employee a Sign-On Bonus of up to a period of three (3) years, subject to the Repayment provision.*
