We are seeking dedicated, passionate individuals who would like the opportunity to be part of a team with a patient-centered approach, which brings decades of experience in providing the most innovative musculoskeletal & orthopedic care. Responsible for coordinating the individualized care of patients throughout the continuum of care services, from abnormal findings to end of treatment, in order to increase and assure timely access to healthcare services.
BS in Nursing preferred. Minimum 5 years clinical nursing experience preferred.
Demonstrates excellent customer service skills including superior telephonic communications skills with the ability to comprehensively assess patient’s physical and psychosocial status as well as conduct complex medical history discussions over the telephone.
Ability to effectively and efficiently coordinate, implement and monitor the patient’s process of care, to communicate openly and empathetically with patients and their family members.
Ability to work independently as well as collaboratively with navigation team and multidisciplinary care team in all aspects of defining and implementing navigation program.
Ability to communicate effectively with diverse audiences including professional staff, patients, community, media, and other service agencies.
Knowledge of adult learning competencies and patient teaching skills.
