The RN Lead demonstrates support of the Company's mission by providing care management/disease management services to clients and families to help in the coordination of care and the management of a patient's individual health needs through and beyond the course of treatment.
* The Lead RN demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient's plan of care while ensuring and facilitating the achievement of quality, clinical outcomes.
Bachelor's Degree in Nursing from an accredited college,
* OCN certification or Case Management certification preferred.
* Three (3) years of clinical oncology experience required.
* One (1)year of experience in Care Management and/or similar role preferred.
* Must possess the ability to utilize a clinical reasoning process for assessing, planning, implementing and evaluating the patient's plan of care while ensuring that coordination of services is done in a timely manner.
* Demonstrates sound knowledge and actions in the care and decision making for the oncology patient population and seeks guidance appropriately.
* Strong computer skills needed, Examples: Microsoft Office, Word, Excel and Outlook required.
