Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Responsible for directing and coordinating all nursing care for critically ill patients based on established clinical nursing practice standards. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Utilizes knowledge of patient's age and cultural diversity in the provision of patient care. Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care.
Up to $25,000 Sign-on Bonus
Initiates assessment of vital signs, orientation to room, allergies, and notification of physician within the timeframe stipulated in unit policy.
Integrates all information available and completes initial patient systems assessment within the time frame established by unit policy.
Recognizes and responds to diagnostic data and initiates appropriate action or intervention as evidenced by documentation reviewed by management
Collects pertinent data using appropriate assessment techniques
Demonstrates ability to provide safe and competent care for patient undergoing invasive hemodynamic monitoring
Able to provide safe nursing care as per standards of care for patients requiring mechanical ventilation
Determines the priority of data collection based on the patient's immediate condition or needs.
Involves the patient, significant others, and health care providers in data collection when appropriate.
Ensures that the data collection process is systematic and ongoing to identify progress and trends that require intervention.
Outcomes are document in terms of measurable goals
Formulates outcomes mutually with the patient and healthcare providers, when possible
Planning
Initiates a written nursing plan of care upon admission
Develops a plan of care that prescribes interventions to attain expected outcomes
Reviews and evaluates the plan of care, updates, and documents patient's progress toward resolution of problems on every shift as evidenced by chart review.
Develops discharge plan with the physician, patient, and/or family and other available resources during the hospital stay.
Ensures that the plan of care reflects current nursing practice
Documents legibly pertinent information in the patients record
Implementation
Initiates, administers and observes intravenous therapy according to hospital policy and procedure throughout the shift.
Assists physicians with the insertion of invasive monitoring equipment, obtains readings, documents and determines significance of readings.
Provides individualized instructions to patient and/or significant others regarding the plan of care on an on-going basis with written documentation of patient's and/or significant other's ability to verbalize
Implements nursing orders autonomously on a daily basis
Provides psychological support and motivation to enable patients to function at their optimal level of independence as observed and documented.
Sets priorities for direct patient care as well as assisting other nurses in establishing priorities.
Determines level of accomplishment of expected outcomes on a daily basis as evidenced by patient progress record and plan of care.
Readjusts plan of care based on evaluation of the patients'/families' needs.
Recognizes the need for aggressive discharge planning based on patient's needs as evidenced by interactions with physicians, Social Services, and other resources.
Evaluates the patient's care and progress through reviewing of patient progress record and plan of care
Utilizes ongoing assessment data to revise nursing diagnoses/patient problems, outcomes and the plan of care as needed.
Involves the patient, significant others, and health care providers in the evaluation process, when appropriate
Consistently maintain complete confidentiality of all medical, financial, employees, computer and other sensitive materials, which may violate HIPAA regulations and/or jeopardize the privacy of others.
Initiates and directs appropriate interventions as it relates to patient care
Demonstrates ability to organize and prioritize assigned responsibilities of patient care on a daily basis.
Demonstrates the ability to make shift assignments based on patient needs and allocated staff when assuming resource duties
Actively participates in developing and maintaining performance improvement activities on the unit and hospital wide
Acquires and maintains current knowledge in nursing practice
Develops cooperative/therapeutic relationship with the patient and/or significant others and projects a positive cooperative and courteous image.
Takes an active role in the unit's function by assuming control and following through on assigned responsibilities.
Utilizes the nursing process to problem solve and informs the Nursing Manager or designee of outcome and/or if follow-up is required.
Supervision Received:
Reports directly to the Unit Manager/Director
Graduate from an accredited school of nursing, Bachelor's degree (BSN) preferred. Minimum 2 years recent Critical Care Nursing (ICU) experience preferred
